*REFERRALS VIA GP ONLY* Mental Health Community Connectors are a team based in community settings in Rotherham who support patients within Primary Care who are living with a Severe Mental Illness. They support patients to access community services, activities, and support as well as for support to improve their physical health and access their physical health checks. The service is accessed by a referral through Primary Care, this is by accessing mental health support at a GP practice as well as the Primary Care Integrated Mental Health Hub (RDaSH). The service is available for those 16+.

Rush House offers accommodation and wrap around support to those deemed as homeless / inappropriately accommodated young people by Rotherham Borough Council aged 16-25. We also offer accommodation to LAC / care leavers within the Rotherham Borough. Further services Rush house offer is support to access EET (Education / employment / training) for those ages 16+ who reside in Rotherham. Open 24/7 365 days a year.
